DESCRIPTION

                             sunitinib 12.5mg  is sold under the brand name sunitinib 12.5mg capsules which is needed to treat a gastrointestinal stromal tumor (GIST) after other medicines (e.g., imatinib) did not work very well.

INDICATION

                                          sunitinib 12.5mg is prescribed for the therapy of advanced renal cell carcinoma as well as the treatment of gastrointestinal stromal tumor after disease progression on or intolerance to imatinib mesylate.

MECHANISM OF ACTION

                           sunitinib 12.5mg capsules shows antitumor and antiangiogenic properties by prohibiting multiple receptor tyrosine kinases, containing platelet-derived growth factors (PDGFRα and PDGFRβ), vascular endothelial growth factors (VEGFR1, VEGFR2, and VEGFR3), FMS-like tyrosine kinase-3 (FLT3), colony-stimulating factor type 1 (CSF-1R), and glial cell-line-derived neurotrophic factor receptor (RET).

ADME

                 Duration of maximum plasma concentration of sunitinib is between 6 and 12 hours
High protein binding was 95 % and 90 % respectively.
Mainly metabolized by cytochrome P450 enzyme, CYP3A4
Elimination of Sutent mainly via feces 61% and sunitinib 12.5mg capsules renal excretion is 16% of administrated dose.
Sunitinib half-life is 40 – 60 hours
Mainly active metabolite half-life was 80 – 110 hours

DOSAGE MANAGEMENT

                The prescribed dose for Gastrointestinal Stromal Tumor is 50 mg PO qDay for 4 weeks, continued by 2 weeks drug-free, repeat cycle.
The prescribed dose for Renal Cell Carcinoma is sunitinib 12.5mg capsules 50 mg PO qDay for 4 weeks, followed by 2 weeks drug-free, repeat cycle.

Adjuvant treatment of RCC :

                • Prescribed for the adjuvant sunitinib 12.5mg therapy of adult patients at maximum risk of relapsed RCC following nephrectomy

                • The Prescribed dose is 50 mg PO sunitinib 12.5mg qDay for 4 weeks, continued by 2 weeks drug-free, repeat cycle for a maximum of nine 6-week cycles totally

Pancreatic Neuroendocrine Tumor :

                Necessary for progress, well-differentiated pancreatic neuroendocrine tumor (pNET) in patients with unresectable locally advanced or metastatic disease
The prescribed dose for Pancreatic sunitinib 12.5mg capsules Neuroendocrine Tumor is 37.5 mg PO qDay continuously without a scheduled off-treatment period

PRECAUSIONS

               • While using of Sutent, Hypothyroidism occurred; check the baseline thyroid function in patients with hypothyroidism or hyperthyroidism and treat per standard medical practice before to initiating treatment

               • While using of sunitinib 12.5mg capsules will have Impaired wound healing resulted; temporary interfere with treatment recommended if go through major surgical procedures

               • When treatment sunitinib 12.5mg will causes severe hepatotoxicity involving liver failure; check liver function tests before starting treatment.

               • When treatment sunitinib 12.5mgHaemorrhagic events involving tumor-related haemorrhage resulted; test the serial complete blood counts and physical examinations

              • When treatment sunitinib 12.5mg, Adrenal haemorrhage occurred in animal studies; check the adrenal function in case of stress such as surgery, trauma or severe infection

             • While on treatment with sunitinib 12.5mg capsules, regulate urine protein; prohibit treatment for 24-hr urine protein ≥3 g; discontinue for repeat episodes of protein ≥3 g despite dose reductions or nephrotic syndrome

SIDE EFFECT 

Common side effects :

                                Hives, Breathing difficulty, Sore throat, Skin pain, Red or Purple skin rash Upper stomach pain, Itching, Tried feeling, Decreased appetite, Dark urine, Jaundice.

Serious side effects :

                             Tiredness, decreased urination, sunitinib 12.5mg Slow heart rate. Thyroid problem Swelling in ankles or feet, Shortness of breath, Unusual bruising, Muscle cramps, Fast heart beats, Sudden dizziness.
